I hereby authorize the above listed Company Advertisement for the Cy-Fair Houston Chamber of Commerce Business Expo 2003 program. I understand that all artwork must be “Camera Ready” and the Cy-Fair Houston Chamber is not responsible for ad design work. Ads are sold on a first come first serve basis and for Chamber Members only. All ads must be prepaid. Deadline for “Camera Ready” artwork is Friday, August 6, 2004.
